Sequoia Transitional Care, a beautiful and welcoming 99-bed skilled nursing facility, is hiring a dedicated LVN Charge Nurse for our Part-Time shift.

Key Responsibilities:


* Provide direct nursing care to residents in accordance with individualized care plans


* Serve as the Charge Nurse during NOC shift, overseeing CNAs and ensuring high-quality resident care


* Administer medications and treatments as prescribed


* Monitor residents for changes in condition and report to RN Supervisor or physician


* Document all care provided accurately and timely in EMR system


* Communicate effectively with residents, families, and interdisciplinary team


* Maintain compliance with safety, infection control, and facility policies


* Respond to emergencies and follow established protocols


* Participate in shift handoffs, training, and care planning

Qualifications:


* Current California LVN license in good standing


* CPR certification


* Experience in a SNF or post-acute setting preferred, but not required


* New graduates encouraged to apply - we will train you!


* Strong written and verbal communication skills


* Computer literate and comfortable with EMR systems


* Compassionate, dependable, and a team player


* Must be able to provide recent medical clearance to work in SNF


* Must pass background check and be eligible to work in the U.S.
