About the Role

Oakwood Gardens Care Center is seeking a skilled and detail-oriented MDS Nurse to join our team on a part-time basis.

This position plays a critical role in ensuring accurate MDS assessments, compliance with regulatory standards, and oversight of resident care planning.

The MDS Nurse provides leadership in the coordination, completion, and submission of MDS assessments, facilitates PDPM meetings, and serves as a resource and trainer for staff.

This role is ideal for a nurse with strong organizational skills, a solid background in long-term care, and a passion for quality resident outcomes.
Key Responsibilities


* Oversee weekly PDPM meetings for skilled patients.


* Track and monitor utilization of PDPM tools (e.g., Physician Diagnosis Verification, Admission/Discharge CSR(GG), Weekly CSR, PDPM Weekly Tracking Tool, Point Right).


* Monitor and analyze QM and QASP performance monthly/quarterly and provide clinical recommendations.


* Train and support new MDS Coordinators on PDPM and MDS processes.


* Accurately assess and determine resident health status and level of care for all new admissions.


* Ensure timely and accurate completion of MDS Assessments (PPS Medicare, quarterly, annual, significant change).


* Coordinate interdisciplinary participation in MDS completion and care planning.


* Conduct resident care conferences in compliance with state and federal regulations.


* Maintain an accurate assessment schedule and ensure thorough documentation.


* Submit MDS assessments electronically, verify accuracy, and perform corrections as needed.


* Ensure all care plans are individualized, updated, and evaluated for outcomes.


* Manage and communicate resident level of care changes, completing required forms and notifying appropriate parties.


* Maintain confidentiality of all resident care information, including protected health information (PHI).

Supervisory Responsibilities


* This position includes supervisory duties related to MDS processes and interdisciplinary coordination.

Qualifications


* Graduate of an Accredited School of Nursing (BSN preferred).


* Current State Board of Nursing license in good standing with all required CE/licensing.


* 3 years of supervisory experience preferred.


* Long-Term Care experience strongly preferred.


* Proficiency in Microsoft Suite; knowledge of PCC preferred.


* Strong decision-making, organizational, and communication skills.


* Ability to work tactfully with residents, families, staff, and regulatory personnel.

* Ability to lift/move up to 25 lbs occasionally.


* Frequent sitting, standing, walking, reading, and computer use.
