Job Duties:


* Develop and implement personalized physical therapy treatment plans for residents.


* Assist with the new sensory program, focusing on resident engagement and sensory stimulation.


* Collaborate with interdisciplinary teams to ensure comprehensive resident care.


* Conduct evaluations to determine therapy needs and document progress.


* Educate residents and their families on at-home exercises and wellness strategies.


* Maintain accurate records and comply with all facility policies and procedures.

Qualifications:


* Current state licensure as a Physical Therapist.


* Strong communication and collaboration skills.


* Passion for working with residents in a post-acute care setting.


* Experience with sensory programs is a plus!
