Social Worker - Adult Inpatient, Advanced Heart Failure/VAD/Transplant - UH

Work Schedule :

Full-time, 100% FTE day shift.

Monday - Friday between the hours of 8:00am - 4:30pm, no weekends required.

Hours may vary based on the operational needs of the department.

We are seeking a Social Worker - Advanced Heart Failure, VAD and Transplant to:



* Provide psychosocial assessment, intervention and support to hospital inpatients and their social supports.


* Provide discharge planning support, connect patients with community and government programs to support resolution of psychosocial risks and barriers to VAD placement/Transplant and discharge from the hospital, and support overall health and wellbeing.


* Intervene with support systems that are caring for patients with complex medical needs including advanced heart failure, provide support, crisis management, and education as indicated.


* Support the Social Work and Transplant teams via case consultation, coverage support, and as a partner in improvement projects, as indicated.


* Option to participate in clinical supervision to elevate to advanced clinical licensure status in the field of Social Work.
