Speech Language Pathologist

Paul's PACE program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community.
The healthcare teams at St.
Paul’s PACE provide a hands-on approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone.
This is a Part Time position.
Typical working hours are between 8:00AM - 4:30PM, Monday - Friday.
Job Summary: We are seeking a qualified and compassionate Speech-Language Pathologist to assist with multiple PACE site locations.
The SLP will support Participants with a variety of communication, cognitive, and swallowing needs, delivering person-centered care across multiple settings.
Duties and Responsibilities:
* Evaluate Participants for speech, language, voice, cognitive-communication, swallowing, and hearing needs per ASHA guidelines
* Develop individualized treatment plans to address impairments, promote independence, and support Participants in achieving their highest functional level.
* Regularly assess progress and modify goals and interventions as appropriate.
* Collaborate with the Interdisciplinary Team (IDT) to ensure communication, cognitive, and swallowing needs are fully addressed.
* Provide training and education to family members, caregivers, and facility staff to support therapy carryover, including use of AAC devices, dietary modifications, and safety practices.
* Guide Participants and caregivers in using and maintaining AAC and adaptive communication tools.
* Troubleshoot malfunctioning AAC equipment and coordinate repairs or replacements as necessary.
* Accurately document device trials, caregiver training, and equipment loans per departmental procedures.
* Deliver direct therapy in areas including expressive/receptive language, articulation, dysphagia management, cognitive-linguistic rehabilitation, and voice therapy.
* Provide visual supports, written home programs, and caregiver instruction to reinforce therapy outcomes.
* Conduct home and community-based visits as needed to complete assessments and deliver education or therapy, documenting all activities in Participant records.
* Collaborate with skilled nursing and assisted living facility staff as needed.
* Participate in training and in-service education for staff and caregivers.
Qualifications and Requirements:
Education/ and Experience:
* Master's degree in Speech-Language Pathology from an ASHA-accredited program
* Valid California Speech-Language Pathology License
* Current BLS certification from an American Heart Association-approved provider
