What You'll Do


* Provide skilled therapy services to residents in a post-acute/SNF setting


* Create and implement individualized treatment plans to maximize outcomes


* Collaborate with nurses, physicians, and other healthcare providers


* Accurately document resident progress and treatment sessions

What You'll Bring


* Active California license or certification in your discipline (PT, OT, SLP, PTA, or COTA)


* Compassion for working with seniors and rehabilitation patients


* Strong communication and teamwork skills


* Previous SNF or post-acute experience preferred, but new grads welcome
