Position Overview:
As a Physical Therapist at Santa Rosa Post Acute, you will be responsible for assessing and treating patients to help them regain physical function and improve mobility.

Working within a collaborative healthcare team, you'll design and implement individualized treatment plans that promote independence and enhance the quality of life for our residents.

Key Responsibilities:


* Perform thorough evaluations to assess patients' physical abilities and develop personalized treatment plans.


* Provide physical therapy interventions to improve strength, mobility, and functional abilities.


* Educate patients and their families on therapeutic exercises, safe movement techniques, and home modifications.


* Collaborate with the interdisciplinary team, including occupational therapists, nurses, and physicians, to coordinate care and optimize patient outcomes.


* Monitor patient progress and adjust treatment plans as necessary to ensure the best possible outcomes.


* Maintain accurate and timely documentation of patient assessments, progress notes, and treatment plans in accordance with facility policies.


* Ensure that care plans align with regulatory requirements and best practices in physical therapy.

Qualifications:


* Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.


* Valid Physical Therapy license in [State].


* Experience working in a skilled nursing facility or similar healthcare setting preferred.


* Knowledge of physical therapy techniques, therapeutic modalities, and adaptive equipment.


* Excellent communication, interpersonal, and problem-solving skills.
