? What You'll Do:


* Assist with resident admissions, discharges, and care planning


* Support residents and families through transitions and emotional needs


* Coordinate with nursing, therapy, and administrative teams to ensure holistic care


* Maintain accurate documentation in accordance with state and federal guidelines


* Help plan and implement programs that enhance quality of life


* Advocate for residents' rights and help resolve concerns compassionately

What We're Looking For:


* A heart for service and empathy for residents


* Experience in social services or a related field (preferred)


* Strong organizational and communication skills


* Familiarity with long-term care or skilled nursing environments is a plus
