Job Title: Caregiver - Assisted Living Facility

Duties:


* Coordinate and monitor individualized care plans for residents and ensuring it is followed


* Communicate with residents, families, and medical professionals to ensure quality of care


* supervise and support care staff to maintain high standards of service


* conduct assessments and participate in care plan meetings


* monitor residents' health status and advocate for timely interventions


* Ensure compliance facility policies to ensure compliance of state regulations

Qualifications:


* Effective communication and engagement skills.


* Attention to detail in monitoring and reporting changes in the environment.


* Strong interpersonal and communication skills


* Ability to prioritize and manage multiple responsibilities
