We're hiring a Resident Care Manager (RCM) to lead clinical care and ensure excellent outcomes for our residents.

Responsibilities:


* Coordinate and implement individualized care plans


* Supervise and support LPNs and CNAs


* Oversee admissions, discharges, and ongoing care documentation


* Participate in care conferences and quality improvement initiatives


* Monitor resident conditions and follow up on changes


* Maintain compliance with state and federal regulations


* Assist with infection control, audits, and staff training

Requirements:


* Active LPN or RN license (required)


* Experience in skilled nursing or long-term care preferred


* Strong leadership, clinical, and communication skills
