We are excited to offer a fantastic opportunity to join our team as a Support Coordinator, where you'll support customers with complex mental health needs to live independently in the community.

This role is within our STEP (Supporting Tenancies, Enabling People) service, based in Sunderland and Washington, focusing on providing tailored, flexible support to help individuals achieve their goals and aspirations.

As part of the growing STEP service in the Northeast, you’ll play a key role in its development and success.

You'll work closely with external agencies and professionals to deliver personalized support plans, empowering customers to build the skills they need for independent living.

Typical day as our Support Coordinator


* Creating support plans with your customers and working together as a team to ensure the smooth delivery of support.


* Supporting our customers with complex mental health needs, learning difficulties and/or dual diagnosis.

Attending MDT's, risk planning meetings etc to enable us to best support our customers.


* Helping customers with daily living activities such as making meals, shopping, budgeting, maintaining their tenancy and moving onto independent accommodation if they are able.


* Carrying out risk assessments, support planning, goal setting and regular reviews.

Cuppas, chats and catch ups.  There’s no rushing from one customer to the next here!  Plenty of time to collaborate with colleagues too 

You bring


* You’re passionate and motivated to help vulnerable people make positive and lasting changes to their lives.

You’ll impress us with your ability to work on your own initiative, positively influencing people from all different backgrounds.


* You have experience in safeguarding, risk assessments, confidentiality, support plans, alongside working collaboratively.


* If you’re not the biggest fan of technology, that’s not a problem to us.

You do need to know that we are quite a high-tech company and most of our application process and mandatory learning are online.

However, we’re glad to support you if you need help.  


* To make sure our customers are well supported, it’s important that you’ve had a job in a similar environment before.

Don’t worry, we won’t drop you in the deep end, we’ll help to you understand how things work at Home Group!


* As this is a community-based job, you need to be able to drive, as well as having access to a vehicle insured for business purposes (we pay your expenses to cover this use!)
