Physical Therapy Assistant

Shadelands Post Acute Rehabilitation Center is seeking a dedicated and passionate Physical Therapist Assistant to join our team on a full-time basis.

Our facility focuses on delivering the highest quality care to our patients, with a current patient base that includes 59 Medicare patients and no long-term care residents.

As a member of our rehab team, you'll be part of a collaborative environment where we strive for excellence in patient care.

Key Responsibilities:


* Treat patients as directed by Physical Therapist.


* Record daily treatment notes and weekly progress notes per PT Board.


* Assist in maintaining department.

Participate in Patient Care and Rehabilitation Conferences, as needed.


* Assist with cleaning and maintenance of treatment area.


* Treat patients per the physician treatment plan.


* Assist nursing department with training of Restorative Aides.


* Communicate with supervisor and other health team members regarding patient progress, problem and plans.


* Participate in in services training program for other staff in the facility.

Record treatment changes per policy and procedures.

Qualifications:


* Active physical therapist assistant license
