100% FTE, Monday - Friday, 8 hour shifts between 7:30am - 4:30pm.

No weekends or holidays.

Hours may vary based on the operational needs of the department.

We are seeking a Nurse (RN) Case Manager:


* Bring a strong clinical background, case management experience and well-developed assessment and critical thinking skills


* Assist with anticipating the discharge needs of post-surgical patients to ensure timely discharge and access to the best possible resources


* Have an understanding of medical necessity, level of care and patient status (inpatient vs outpatient).


* Knowledge of community resources such as transportation, skilled nursing facilities or home health agencies.

Education :

Minimum - Bachelor's degree in Nursing.

Equivalent combination of education and experience will be considered.

Preferred - Master's degree in Nursing or healthcare related field.

Work Experience :

Minimum - Three (3) years of relevant clinical nursing experience.

Preferred - Recent experience as a hospital-based Nurse Case Manager.

Licenses and Certifications :

Minimum - RN license in the state of Wisconsin

Preferred - ACMA certification as a Case Manager
