MDS Coordinator

Wage Range: $45/hr-$55/hr

Job Type: Full-Time

Reports To: Director of Nursing

Job Summary: The MDS (Minimum Data Set) Coordinator is responsible for overseeing the MDS assessment process in long-term care facilities.

This role ensures compliance with federal and state regulations, maintains accurate patient records, and coordinates with interdisciplinary teams to develop and implement individualized care plans.

Key Responsibilities:


* Assessment and Documentation:
+ Complete and update MDS forms for all residents in the facility.
+ Perform clinical assessments to gather comprehensive information on residents' health status.
+ Ensure accurate and timely submission of MDS data to maintain compliance with Medicare and Medicaid requirements.


* Care Planning:
+ Develop and update individualized care plans based on MDS assessments.
+ Coordinate with nursing staff, physicians, and other healthcare professionals to implement care plans.
+ Monitor and evaluate the effectiveness of care plans and make necessary adjustments.


* Compliance and Quality Assurance:
+ Ensure all MDS documentation meets federal and state regulations.
+ Participate in quality assurance and performance improvement activities.
+ Conduct audits and reviews of MDS documentation to ensure accuracy and completeness.


* Communication and Collaboration:
+ Facilitate interdisciplinary team meetings to discuss resident care and progress.
+ Communicate with residents and their families to explain care plans and address concerns.
+ Serve as a resource for staff regarding MDS processes and regulations.

Qualifications:


* Education:
+ Registered Nurse (RN) preferred or Licensed Practical Nurse (LPN) with a valid state license.
+ Certification in MDS (e.g., RAC-CT) preferred.


* Experience:
+ Minimum of 2 years of experience in a long-term care setting.
+ Previous experience with MDS assessments and care planning.


* Skills:
+ Strong knowledge of MDS 3.0 and federal/state regulations.
+ Excellent organizational and time management skills.
+ Effective communication and interpersonal skills.
+ Proficiency in electronic health records (EHR) systems.

Work Environment:


* Primarily works in a long-term care facility.


* May require occasional evening or weekend shifts to meet the needs of residents.
