The Opportunity


* Seeking Board Eligible/Certified Family Practice or Emergency Medicine trained physicians.


* Current IN state license is a plus.

Community Hospital Outpatient Center, St.

John - St.

John, Indiana


* These outpatient centers serve the local community, both appointments and walk-ins.


* Additional services include heart care, immediate care, laboratory and therapy services, among others.


* Minor procedures performed at both locations.


* Single provider coverage.


* Will be cross-credentialed at both sites.


* Typical hours of operation are Monday through Friday, 8am-8pm, and Saturday and Sunday, 8am-4:30pm.
