Job Requirements:


* Current Indiana QMA Certification


* Current BLS/CPR Certification (Online-Only Certifications not-accepted)


* You may also be asked to cover CNA openings at a facility (if you are willing); CNA pay rates are $22/hr


* 1 full year of Long Term Care experience within the last 3 years


* Physical within the last 6 months that states you are "free from" or "without evidence of" communicable diseases


* Current PPD or Chest X-ray 


* 2 Professional References (RN's or LPN's)


* Clear Criminal History Report (we will obtain at no cost to you)
