Key Responsibilities:


* Evaluate patients and develop individualized treatment plans to improve physical function and mobility


* Provide therapeutic exercises, manual therapy, and education to patients


* Work closely with an interdisciplinary team to ensure comprehensive patient care


* Monitor progress and adjust treatment plans as necessary


* Maintain accurate and timely patient documentation

Qualifications:


* Valid CA Physical Therapist license


* Experience in a skilled nursing or similar healthcare setting preferred


* Strong communication and collaboration skills
