Support Coordinator

Support Coordinator (Outreach)

Based in communities around Newcastle.

Permanent, full time (37.5 hpw) Based on rota.

To be flexible to work shifts sleep ins or waking nights, including weekends and Bank Holidays.

Pay £12.61 per hour (£24,664 per annum) and great benefits including Health Cash Plan.

Driver preferred but not essential.

Typical day as a lead Support Worker (known by us as Coordinator)


* Creating support plans with your customers (and coordinating other support plans for the service). 


* You will empower customers to develop independent living skills they need, to successfully move on (if and when ready).


* You will offer support so that customers are able to live fulfilled lives in the community with a package of support.


* Working closely with external agencies so that there is a seamless approach to support.


* Carrying out risk assessments, support planning, goal setting and regular reviews.


* Support clients with housing benefit applications and ensuring that our clients are in receipt of the correct benefits.


* Supporting with any tenancy related issues reporting repairs, contacting utility companies etc.

You bring


* Passion to support our customers to live their best life, working collaboratively with an eye for detail. 


* Experience of creating person-centred support plans and supporting colleagues with this.


* Experience of coordinating and assessing customer referrals.


* The ability to work on your own initiative, remain calm under pressure and have a resilient approach.


* Experienced in safeguarding, risk assessments, confidentiality and data protection.


* IT skills: It's important that you are technologically savvy (or at the very least not a technophobe!)
