Staff Position Description

Position Purpose:

Under the general supervision of the Clinical Director, and clinical leadership of the Registered Nurse (RN), and in collaboration with the clinical team, the LNA performs clinical nursing procedures and functions within the Arizona (AZ) LNA Scope of Practice.

The incumbent supports the plan of care for each patient, as determined by the health care team, and in accordance with the goals of the organization.

Works to improve patient safety and patient experience.

Licensed Nursing Assistant:

· Responsible for attending to the daily living and safety needs of an assigned group of patients.

· Assist registered nursing personnel in the care of patients by performing various patient care activities and related non-professional services.

· Collaborates and coordinates with other departments to provide timely effective care consistent with individual’s needs, choices and preferences.

· Obtains and records vital signs, heights and weights per policy.

· Meets patient’s personal hygiene and basic needs in a timely manner.

· Ensures that the patient is comfortable, necessary supplies are available and in reach.

· Keeps patient environment safe and tidy.

· Provides patient specific care as appropriate to age of patient.

· Demonstrates effective communication skills with children, adolescents, adults and geriatric patients to include use of appropriate therapies such as play, written, and picture board. 

· Reports changes in patient’s condition, out of normal values, patient/family concerns or complaints to nurse, charge nurse and/or supervisor.

· Applies AIDET as well as other evidence-based behaviors.

· Participates in developmental and organizational initiatives for process improvement and quality assurance as requested.

· Follows appropriate infection control procedures.

· Performs and accurately documents the following procedures as directed by the RN / LPN:

a.

Blood glucose checks and documentation

b.

Discontinuation of urinary catheters

c.

Discontinuation of peripheral IVs

d.

Performs skin care interventions and treatments, as permitted by state regulation.

· Maintains patient and staff confidentiality.

· Assists in the orientation of new staff as requested.

· Performs other duties as assigned.
