RN - Ambulatory

Work Schedule :

80-100% FTE, 8 hour shifts between clinic hours of 7:30 am - 5:30 pm, Monday - Friday.

No weekends or holidays.

Hybrid position with option to work a combination of remote and onsite workdays after successful department orientation.

RNs will conduct onsite and telehealth Medicare Annual Wellness visits with adult primary care patients.

We are seeking Registered Nurses (RN) to join the new Medicare Annual Wellness Program


* Work collaboratively with primary care teams to ensure appropriate risk assessment for Medicare enrollees leading to positive outcomes of care.


* Update patient information, reconcile medications, perform chronic condition screenings and identify health risks which follow-up care.


* Promote healthy lifestyles, wellness education and provide education about chronic conditions.


* Provide compassionate care in a patient and family-centered setting.

Qualifications



* Graduate of school of nursing Required


* Bachelor's Degree in Nursing (BSN) Preferred

Work Experience



* 6 months of RN experience Required


* Relevant RN experience Preferred

Licenses & Certifications



* Registration as a registered nurse in the state where employed or licensure in a state in the licensure compact Upon Hire Required


* CPR/BLS Certification Upon Hire Required


* Applicable clinical certification Preferred
