ABOUT THE POSITION

Social Services Practitioner II/III

Adolescent Family Life Program (AFLP)

The County of Riverside's Public Health department has an opening for Social Services Practitioner III with the Adolescent Family Life Program (AFLP).

The program aims to support and empower expectant/pregnant and parenting youth by increasing access to utilization of needed services, increasing social and emotional support, building resiliency, empowering youth to make informed decisions about their sexual reproductive health, and strengthening youth knowledge for education and career attainment.

This position will work with expectant/pregnant and parenting youth up to 21 years of age.

This position will conduct home visits with pregnant and parenting adolescents.

The position will provide case management services by delivering a model called Positive Youth Development (PYD), assist teens with accessing community resources, educate pregnant/parenting adolescents regarding many issues such as role as a parent, care of their infant/child, and goal setting.

The position will assist and support teens in attending school to complete at least their HS diploma, help teens with setting future goals and developing healthy relationships, support adolescent parents with life planning and many other supports based on the PYD model.

This position will require driving therefore a California Drivers License is required and must be maintained throughout employment .

Spanish bilingual candidates who possess experience in case management and working with adolescents are highly desired.
This position will work a 9/80 schedule.

Under general direction, performs difficult social service casework dealing with complex individual and family problems or social service casework for patients and their families in healthcare facilities; plans, organizes, implements, and evaluates all clinical services provided to pediatric patients and needs of children in required assignments; may participate in and assist with conducting in-service training programs; may provide leadership as a program consultant in the utilization of casework concepts and methodology; performs other related duties as required.

The Social Services Practitioner Ill is a broad range journey to advanced level classification of the professionally trained social caseworker in the Social Services Practitioner series.

Incumbents are characterized by the responsibility for performing full scope social service casework often requiring difficult diagnosis and intensive treatment services.

The Social Services Practitioner Ill is distinguished from the Social Services Supervisor series by the latter's responsibility to provide full time supervision of staff performing casework.
