General Purpose

Position Summary
The Physical Therapist evaluates, develops, and delivers individualized treatment plans to help residents restore function, improve mobility, and achieve their highest level of independence.
The PT also provides staff training, supervises PTAs, and ensures accurate documentation.
Key Responsibilities
* Perform evaluations within 24 hours or 1 business day of physician referral
* Develop treatment plans and obtain physician approval
* Provide direct treatment according to the care plan
* Supervise Physical Therapist Assistants (PTAs) per state practice act
* Document evaluations, daily notes, progress notes, and recertifications in compliance with PT Board and payer requirements
* Participate in care conferences, utilization reviews, and interdisciplinary meetings
* Provide caregiver and family training for safe discharge planning
* Recommend necessary DME (braces, walkers, canes, grab bars, etc.)
* Assist with Restorative Aide training
* Maintain equipment and report any issues
* Support facility in-service programs and staff education
Qualifications
* Bachelor's, Master's, or Doctorate in Physical Therapy
* Active California PT license in good standing
* Strong communication skills and the ability to collaborate with residents, families, and staff
* Proficiency with EMR systems (Casamba, PointClickCare preferred)
