Position: Full-Time LVN
Location: All Saints Subacute & Transitional Care - San Leandro, CA
Pay Rate: $34.00 - $35.00 per hour

Key Responsibilities


* Provide direct nursing care, including medication administration, treatments, and resident assessments


* Monitor resident conditions and promptly report changes to physicians and supervising nurses


* Collaborate with interdisciplinary teams, including therapy staff and physicians, to support individualized care plans


* Accurately document care and resident progress in electronic medical records (PointClickCare experience preferred)


* Supervise and support Certified Nursing Assistants (CNAs) to ensure quality resident care


* Serve as a compassionate advocate for residents and their families

Qualifications


* Current and active California Licensed Vocational Nurse (LVN) license


* Current CPR/BLS certification


* Previous experience in a skilled nursing, subacute, rehabilitation, or post-acute setting preferred


* Strong clinical assessment, communication, and organizational skills


* Ability to work effectively as part of a multidisciplinary team
