Pay Range: $40-$45/hr BOE
San Joaquin Nursing and Rehab is seeking an experienced and detail-oriented Admissions Nurse ( RN) to join our team on a part-time basis for the 3-11 PM shift.
This role is critical in ensuring a smooth, welcoming, and compliant admission process for new residents entering our skilled nursing facility.
General Purpose
The Admissions Nurse is responsible for coordinating and completing resident admissions while ensuring all clinical documentation, assessments, and regulatory requirements are met.
This position works closely with nursing leadership, admissions, and interdisciplinary team members to support quality resident care and compliance.
Essential Duties

Clinical & Admissions Functions
* Complete initial nursing assessments and admission documentation for new residents.
* Review hospital paperwork, physician orders, and medication reconciliation for accuracy and completeness.
* Ensure admission documentation is completed timely and in compliance with federal, state, and facility guidelines.
* Communicate effectively with physicians, families, hospitals, and interdisciplinary team members.
* Initiate care plans and ensure appropriate handoff to nursing staff.
* Ensure accurate entry of resident information into PointClickCare or other clinical systems.
* Provide education and orientation to residents and families upon admission.
Qualifications
* Current LVN or RN license in good standing (Required)
* Skilled Nursing Facility (SNF) or post-acute experience preferred
* Strong assessment, documentation, and organizational skills
* Ability to work independently during evening hours
* Familiarity with PointClickCare preferred
