Visalia Post Acute is seeking a compassionate and skilled Occupational Therapist (OT) to join our dedicated rehabilitation team.
This full-time opportunity is ideal for an OT who is passionate about helping residents achieve maximum independence, function, and quality of life in a skilled nursing and rehabilitation setting.
General Purpose
The Occupational Therapist (OT) evaluates and treats residents, develops and implements individualized treatment plans, communicates with physicians, families, and interdisciplinary team members, and maintains accurate and timely documentation in the medical record.
The OT also provides clinical guidance and education to staff to support optimal resident outcomes.
Essential Duties and Responsibilities
* Evaluate residents within 24 hours (or one business day) of physician referral; document delays appropriately
* Develop individualized treatment plans and obtain physician approval for services
* Provide occupational therapy treatment per the physician-approved plan of care
* Supervise Certified Occupational Therapy Assistants (COTAs), aides, and students in accordance with the state practice act
* Communicate resident progress, challenges, and plans with the rehab and interdisciplinary teams
* Participate in patient care conferences, utilization review meetings, and rehab meetings as needed
* Complete evaluations, daily treatment notes, weekly progress notes, recertifications, and discharge documentation per regulatory and payer requirements
* Educate residents, families, and caregivers on safety, adaptive techniques, and home programs
* Participate in discharge planning and coordination of care
* Recommend and facilitate ordering of durable medical equipment (DME) to promote functional independence
* Conduct in-services and training for facility staff as assigned
* Report equipment issues promptly to ensure safe and effective operation
* Maintain professional, respectful relationships with residents, families, and staff

General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by
your supervisors.
Essential Duties
• Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.
• Use only authorized abbreviations established by this facility when recording information.
• Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
• Report all accidents and incidents you observe on the shift that they occur.
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access facility's information system.
• Perform only those nursing care procedures that you have been trained to do.
• Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).
• Greet residents and escort them to their room.
• Introduce resident to his/her roommate, if any, and other residents and personnel as appropriate.
