About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks.
This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.
Our providers:
* Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
* Administer and order point of care and lab-based testing as needed
* Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
* Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
* Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities

* Flexibility – Visits are scheduled based on YOUR availability.
We conduct assessments Sunday – Saturday from 8 am – 7 pm.
You can work during the day, evening, and/or weekend based on your schedule.

About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks.
This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.
Our providers:
* Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
* Administer and order point of care and lab-based testing as needed
* Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate
* Collaborate with Primary Care Physician (PCP) on patient education and follow up as necessary
* Provide services at the member’s place of residence which can be in their homes or Skilled Nursing Facilities

* Flexibility – Visits are scheduled based on YOUR availability.
We conduct assessments Sunday – Saturday from 8 am – 7 pm.
You can work during the day, evening, and/or weekend based on your schedule.

About the Position:
* We have an opportunity in our Yakima, Washington location for a safety and customer focused Class A CDL Truck Driver who will be responsible for preparing equipment, loading it and returning it from customer sites to our rental yards.
* Our drivers will provide training and/or instructional direction on the safe use of the equipment.
* It is a physically demanding job, requires attention to detail and a strong commitment to safety.
* You must be willing to communicate and cooperate with customers, respond to their questions, and safely operate delivery vehicle in compliance with company and DOT Guidelines, Rules and Regulations.
* Previous experience in heavy equipment delivery is preferred and in the rental industry is desired.

Qualifications & Experience Needed:
* A CDL- Class A is required.
* Air brake endorsement is required.
* Experience driving a manual truck.
* This job requires at least two years driving experience or have completed a course of instruction from a Truck Driver training school that operates equipment similar to a CDL Truck Driver.
* You must be dependable, responsible, and committed to the safe operation of your vehicle and to the safe and timely pickup and delivery of equipment.

PRIMARY FUNCTION:
The primary function of this position is to perform expert level repairs on material handling equipment by trouble shooting using diagnostic equipment and manuals while working in the field.
ESSENTIAL DUTIES:
* Review the work schedule, check the equipment and supplies required to perform the work scheduled, comply with PPE requirements, and otherwise prepare for duties.
* Use manuals, repair manuals, parts manuals, parts diagrams to troubleshoot engines, engine fuel systems, transmissions, or other components on material handling equipment.
* Use lift trucks, overhead cranes, & other aides to lift or move parts into position to be repaired.
* Use hand & power tools to remove, install, replace, re-build, repair, & test parts/equipment on engines, engine fuel systems, transmissions, & other components.
* Use miscellaneous resources & computers to reference parts, get information about parts & enter data into record systems.
* Use hand & power tools to disassemble/re assemble parts or equipment, drains fluids into tubs, cleans parts with rags, cleaners, power washer.
* Climb up onto, under, & into vehicles, uses hand & power tools to disassemble/re assemble: sheet metal, engines, transmissions, fuel systems, & other parts or equipment.
* Replaces pumps, batteries, radiators, bearings, seals, wiring, solenoids, fuel lines, nozzles, tests float valves, glow plugs, alternators.
* Use overhead hoists to move parts that need to be moved off of & back onto the material handling vehicle.
* Use meters, measuring devices, & computers to test & recalibrate equipment, completes warranty paperwork.
* Perform inspections, do preventative maintenance, change oil, replace filters, belts, coolants, hoses, may drain/clean/refill fuel systems.
MINIMUM REQUIREMENTS:
Education:
Must have a high school education or GED.
Work Experience:
Six years of experience on Yale, Clark or other similar equipment; Fully understands all equipment systems (i.e.
engines, hydraulics, electrical, sub-electrical, transmissions, etc.) for almost all equipment model lines.
Physical:
Must be able to daily and repeatedly stoop, kneel, bend and climb onto and underneath various equipment to perform diagnosis and repairs.
Must be able to effectively see to perform repairs, research technical manuals, and develop written repair work order reports.

About the Role
During a visit that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment and other risks.
This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment (CHA) helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit.

* Conduct adult / geriatric assessments to include medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols
* Administer and order point of care and lab-based testing as needed
