Job Summary:
As a Direct Support Professional (DSP) in our Crisis program, staff must be comfortable responding to interfering behaviors and implementing physical intervention techniques daily.
In addition to responding to interfering behaviors daily, staff will also assist individuals in working toward their personal goals. DSP staff support our persons served with tasks such as self care, community outings, homework, shopping, cleaning, attending appointments, and taking medication.
Staff should feel comfortable providing personal care, medical supports, and other daily personal cares and supports to those they assist. Please note, all staff will be cross trained at 1-2 other programs for training purposes.
Essential Job Duties:
* Respond to interfering behaviors including verbal and physical aggressions.

General Purpose:
Hayward Gardens Post Acute is seeking a dedicated Staff Occupational Therapist (OTR) to join our team.
In this role, you will evaluate and treat residents, communicate with families, physicians, and other health team members, and maintain documentation of services in the medical records.
You will also conduct ongoing in-services and training for facility staff.
Essential Duties:
* Resident Evaluation: Assess residents to determine their occupational therapy needs, functional limitations, and rehabilitation potential in accordance with physician orders and facility policies.
* Treatment Plans: Develop, implement, and update individualized treatment plans to enhance mobility, strength, balance, endurance, and functional independence.
* Training Support: Assist the nursing department with training Restorative Aides.
* Supervision: Supervise Occupational Therapy Assistants in direct resident care and related activities, following state practice acts.
* Communication: Regularly communicate with physicians, nursing staff, interdisciplinary team members, residents, and families regarding progress, goals, and discharge planning.
* Meeting Participation: Participate in Resident Care conferences, Utilization Review meetings, and Rehabilitation meetings as needed.
* In-Service Training: Provide in-service training programs for other staff in the facility as necessary.
* Documentation: Complete timely, accurate, and compliant documentation in accordance with facility, governmental, and third-party payer requirements.
* Safe Environment: Maintain a safe treatment environment by adhering to infection control, safety, and equipment maintenance protocols.
* Education: Educate residents, families, and caregivers on treatment goals, home exercise programs, maintenance programs, safety techniques, and fall prevention strategies.
* Equipment Recommendations: Recommend and facilitate the ordering of necessary durable medical equipment (DME) for residents to support independence in activities of daily living (ADLs), including adaptive equipment, splints, assistive devices, elevated toilet seats, and grab bars.
* Confidentiality: Maintain confidentiality of residents' information as well as company information.
* Equipment Maintenance: Report any problems with departmental equipment to ensure it is maintained in good working order.
* Quality Assurance: Contribute to quality assurance, performance improvement initiatives, and facility rehabilitation program development as assigned.
* Professional Relationships: Relate positively, effectively, and appropriately with residents, families, staff, and professional colleagues.
