The Emerald Peek Rehabilitation & Nursing Center in Peekskill, NY is a sub-acute facility with a full time Physical Therapist opening on our in house rehab team.
Hiring is done on site by our Director of Rehab and facility leadership, not by an outside recruiter.
You will interview with the people you will actually work with.
We are part of a growing network of 35 skilled nursing facilities, which means real clinical support behind you and real places to go from here.
Your Day to Day
* Complete physical therapy evaluations covering strength, range of motion, balance, gait, transfers, endurance, and fall risk
* Set functional, measurable goals tied to each resident's prior level of function and planned discharge setting, under physician direction
* Deliver skilled interventions including therapeutic exercise, gait and transfer training, balance and neuromuscular re education, and functional mobility training
* Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time
* Train residents, families, and caregivers on safe mobility, transfers, assistive device use, and home carryover
* Partner with nursing on safe transfer status, fall prevention, and restorative programming
* Recommend assistive devices, wheelchairs, seating, and durable medical equipment for discharge
* Direct and supervise Physical Therapist Assistants and students in accordance with the state practice act

Lebanon Health and Rehabilitation Center is seeking a Physical Therapist (PT) for our Facility.
Physical Therapist (PT) Duties Include:
The Physical Therapist is responsible for the assessment of referred patients that require rehabilitation services which includes: providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
Also, the Physical Therapist will establish and administer a treatment program with specific goals determined according to the patients capacity and tolerance under the direction of the Physician.
As well, the Physical Therapist must set realistic and achievable goals for their patients, document and record the patients condition and educate patients and families in an appropriate physical therapy method.
Physical Therapist (PT) Requirements/Qualifications:
* Must have graduated from an accredited Physical Therapy college or university.
* Must have a valid professional license and registration for state of practice.
* The Physical Therapist is responsible for overseeing and supervising Physical Therapy Assistants.
* Experience with long-term care and/or geriatric care is preferred but not limited to.
* Well versed with documentation writing
* Evaluations, weekly progress notes, recertification and discharge documentations.
* Collaborates with all disciplines to provide best patient centered treatment and care.
* Exceptional patient service skills and ability to demonstrate clinical excellence
* Report to work on time, adhere to scheduled hours and demonstrate professionalism at all times.
* Perform all other duties assigned by Director of Rehabilitation
* Compliance with confidentiality, HIPPA and Federal Residents Rights.
* Experience with Case Mix Index and PPS scheduling is preferred however not limited to.
