General Purpose
The Occupational Therapist (OT) evaluates and treats patients referred by physicians, develops and implements individualized treatment plans, collaborates with interdisciplinary team members, and maintains accurate and timely clinical documentation in accordance with federal, state, and facility regulations.
Essential Duties and Responsibilities
* Evaluate patients and establish individualized occupational therapy treatment plans
* Treat patients in accordance with physician orders and approved plans of care
* Document evaluations, daily treatment notes, progress notes, recertifications, and discharge summaries per regulatory and payer requirements
* Educate patients and families on adaptive techniques, safety, and functional independence
* Collaborate with nursing, therapy, and interdisciplinary teams to support optimal patient outcomes
* Participate in patient care conferences, rehab meetings, and discharge planning as needed
* Recommend and facilitate ordering of adaptive equipment and durable medical equipment (DME)
* Maintain safe, clean, and organized treatment areas
* Report equipment issues to ensure proper operation and safety
* Maintain patient confidentiality and uphold all resident rights and privacy standards
Qualifications
Education & Experience
* Bachelor's degree in Occupational Therapy required
* Master's or Doctorate degree preferred
* Skilled Nursing Facility or post-acute experience preferred
Licensure & Certifications
* Current, active California Occupational Therapist license in good standing
* Current CPR/BLS certification

General Purpose
The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident's assessment and care plan, and as may be directed by
your supervisors.
Essential Duties
• Use the wristband or photo card file to identify residents before administering treatments, serving meals, etc., as necessary.
• Use only authorized abbreviations established by this facility when recording information.
• Report all changes in the resident's condition to the Nurse Supervisor/Charge Nurse as soon as practical.
• Record all entries on flow sheets, notes, charts, etc., in an informative, descriptive manner.
• Report all accidents and incidents you observe on the shift that they occur.
• Agree not to disclose resident's protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
• Report any known or suspected unauthorized attempt to access facility's information system.
• Perform only those nursing care procedures that you have been trained to do.
• Ensure that the resident's room is ready for receiving the resident (i.e., bed made, name tags up, admission kit available, etc.).

• Follow established policies concerning exposure to blood/body fluids.

• Measure and record temperatures, pulses, and respirations (TPRs) of residents, as instructed.
Weigh and measure residents as instructed.
• Ensure that residents who are unable to call for help are checked frequently.
• Answer resident calls promptly.
• Check residents routinely to ensure that their personal care needs are being met.
• Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on his/her tray, where it is located, if it is hot/cold, etc.).
