Job Description
* Treatment Planning: Develop effective treatment plans and obtain physician approval for services.
* Patient Care: Provide treatment in accordance with established physician treatment plans.
* Training Support: Assist the nursing department in training Restorative Aides and supervise Physical Therapy Assistants in patient care activities.
* Communication: Maintain clear communication with supervisors and healthcare team members regarding patient progress and challenges.
* Meetings Participation: Engage in Patient Care Conferences, Utilization Review meetings, and Rehabilitation Conferences as required.
* Documentation: Accurately record evaluations, daily treatment notes, weekly progress notes, and recertification documentation in accordance with PT Board requirements.
* Adjustments: Document treatment changes following established policies and procedures.
* Family Instruction: Educate patients' families and nursing staff on maintenance programs and caregiver training for discharge preparations.
* Discharge Planning: Contribute to discharge planning processes for patients.

General Purpose
The Nurse Aide in Training is designed for individuals preparing to become Certified Nursing Assistants (CNAs).
Under the supervision of licensed nursing staff, trainees receive hands-on experience and classroom instruction to develop the skills necessary to provide high-quality care to residents in a skilled nursing facility.
Essential Duties
* Participate in approved CNA training program provided by the facility or affiliated institution
* Assist residents with basic care needs such as grooming, dressing, and hygiene under supervision
* Observe and report changes in residents' conditions to licensed staff

General Purpose
The primary responsibility of your job position is to access and treat wounds of patients in the facility during your shift/rounds.
