A Calling with Impact At Lifted Hospice, our Registered Nurse (RN) Case Managers play a vital rolecombining clinical excellence with genuine compassion to provide care that honors life and brings comfort when it matters most.

As an RN Case Manager, youll deliver personalized nursing care in the patients home or care setting.

Youll collaborate closely with the interdisciplinary team to build and follow care plans that reflect the patients goals, support their family, and meet the highest standards of clinical, ethical, and emotional care.

Youll also educate and equip families to help them understand what to expect, how to provide comfort, and how to feel confident in their role during a sacred season of life.

Your Role in the Mission As an RN Case Manager at Lifted Hospice, youll provide expert care and steady support for patients and families facing end-of-life.

Youll lead the bedside experiencemanaging symptoms, guiding care, and helping families feel confident and supported.

Key Responsibilities Assessing needs Completing thorough assessments to guide care based on each patients physical, emotional, and clinical needs.

Managing symptoms Delivering skilled hospice care to help manage pain and improve quality of life.

Working as one team Collaborating with the full care team, the patient, and their loved ones to create and follow a unified care plan.

Keeping clear records Documenting care and updates accurately in line with all standards and requirements.

Supporting families Educating and encouraging caregivers, offering emotional support, and helping families feel prepared and confident.

Physical Requirements & Work Environment Remain active throughout the day Ability to stand, walk, bend, and move frequently while providing hands-on patient care.

Assist with patient mobility Ability to safely lift up to 50 pounds and assist with transferring, repositioning, and supporting patients as needed.

Perform with professionalism under pressure Ability to prioritize responsibilities, make timely decisions, and respond effectively to changing patient and family needs in a fast-paced environment.
