Licensed Practical Nurse

The Admiral at the Lake is seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our Health Services team.

The LPN is responsible for promoting and restoring residents quality of life by providing high-quality nursing care based on individual needs and established care plans.

This role provides clinical support, supervises Care Partners, and works collaboratively with residents, families, and the interdisciplinary team to ensure safe, individualized, and person-centered care.

Assess residents upon admission and periodically to identify changes in health and wellness.

Administer medications and treatments in accordance with licensure, physician orders, and established procedures.

Develop and support individualized, person-centered care plans in collaboration with residents, families, and the interdisciplinary team.

Provide education and emotional support to residents, families, and responsible parties regarding health conditions, medications, and self-care.

Monitor residents for changes in physical, psychological, and social conditions and promptly report concerns to the RN or Director of Nursing.

Supervise and support Care Partners to ensure appropriate care and accurate, timely documentation.

Respond to call bells and emergency situations promptly to support resident safety and well-being.

Participate in care plan meetings and recommend alternatives to improve resident care.

Maintain accurate and timely clinical documentation, medication records, and controlled substance documentation.

Ensure medications, supplies, and equipment are maintained appropriately for resident care and the next shift.

Follow infection control practices, HIPAA regulations, and organizational policies and procedures.

Maintain a clean, safe, and comfortable resident environment.

Complete assigned shift responsibilities and other duties as needed.

Qualifications Graduate of an approved Practical Nursing program.

Current and valid Illinois Licensed Practical Nurse (LPN) license.

Competency in medication administration and treatments required.

Basic Life Support (BLS) certification required.

One year of experience as a Practical Nurse in a Skilled Nursing Facility (SNF) preferred.

Strong communication, organizational, and leadership skills.

Ability to work effectively with residents, families, and interdisciplinary team members.

Ability to remain calm and professional in demanding situations, including emergencies.

Physical Requirements Ability to stand and walk for extended periods throughout the shift.
