Registered Nurse

Heart to Heart is a leading provider in services to individuals with Intellectual and Developmental Disabilities throughout New Jersey and Pennsylvania.

We are currently seeking a Registered Nurse who aligns with the mission and vision of Heart to Heart to work in our program and provide new hire orientation and ongoing trainings to community support staff and LPNs through the Pennsylvania Region.

Must be at least 18 years of age or older Two (2) years working with individuals with intellectual and developmental disabilities (preferred) Two (2) years of nursing experience Valid New Jersey RN License in good standing with Board of Nursing Non-Probationary Driver's License High School Diploma or Equivalent Essential Job DutiesProvides oversight and support for all medical and health care needs of individualsConducts training for management and community support staffProvides oversight/supervision of LPN's working in our group homes and day programsProvides nursing services to individuals by identifying and reporting on health needs of individuals through observation of their physical, social and emotional needs as well as review of their medical recordsAdministers medication and documents in compliance with Heart to Heart policies which conform to ODP guidelinesOther duties as assigned by Chief Program OfficerOrients and trains new community support staff to the medical needs of specific individuals with whom they will be workingEnsures that policies and procedures that guide and support the provision of care are implemented and the recommendations to improve performance of care are implemented within Heart to HeartReviews and maintains the system of clinical records, reporting, reporting forms and service data, ensuring that all documentation is timely, appropriate, accurate and within agency guidelines
