Position Summary

The Social Services & Activities Director is responsible for leading and coordinating the facility's Social Services and Activities programs.

This position plays a key role in supporting residents' emotional, psychosocial, and recreational needs while promoting dignity, choice, and meaningful engagement.

The Director collaborates with residents, families, staff, and community resources to create a supportive environment that enhances residents' overall well-being and quality of life.

Essential Duties and Responsibilities
Social Services

- Develop, implement, evaluate, and direct the facility's social services program.

- Complete psychosocial assessments and maintain accurate documentation.

- Participate in admissions, care planning, discharge planning, and interdisciplinary team meetings.

- Assist residents and families in accessing community resources and supportive services.

- Advocate for resident rights and address resident and family concerns.

- Coordinate responses to reports of missing, lost, or stolen belongings.

- Maintain strong working relationships with community agencies and support organizations.

- Support residents and families through transitions, adjustment periods, and discharge planning processes.

Activities

- Develop, implement, and evaluate a comprehensive activity program that meets the physical, emotional, spiritual, cognitive, social, and cultural needs of residents.
