Case Manager - Behavioral Health ServicesLocation: Allentown, PA
Pay Rate: $20 /hour
Schedule: Full-Time | Monday-Friday, 8:30 AM-4:30 PM

Join Chimes-Holcomb as a Case Manager!In this community-based role, you'll help individuals receiving behavioral health services access essential resources, overcome challenges, and work toward their personal recovery goals.What You'll Do


* Manage a caseload of up to 30 individuals receiving behavioral health services.


* Develop personalized service plans that promote recovery and independence.


* Connect clients with housing, healthcare, employment, and community resources.


* Monitor client progress and provide crisis intervention when needed.


* Collaborate with families, providers, and community support teams.


* Maintain accurate and timely documentation.


* Participate in a rotating 24/7 on-call schedule.

What We're Looking For


* Bachelor's degree in Psychology, Social Work, Criminal Justice, Counseling, Education, or a related field OR


* High school diploma with qualifying social science credits and relevant human services experience.


* Valid Pennsylvania driver's license with no more than 3 points.


* Strong communication, organizational, and problem-solving skills.


* Ability to complete required background clearances.

Additional education or experience may be considered in accordance with program requirements.
