CHR Nurse

Position: CHR Nurse Supervisor: Community Health Center Director Division: Health Services Administration Department: Community Health Representative Program Responsibilities: The CHR Nurse provides licensed nursing services in home, community, and Health Services settings to eligible community members in accordance with the nurses education, licensure, demonstrated competencies, organizational policies, established protocols, and applicable scope of practice.

The position supports the Community Health Representative (CHR) Program through nursing assessment, health monitoring, patient education, care coordination, referrals, follow-up, chronic disease support, and connection to appropriate healthcare resources.

The CHR Nurse works collaboratively with the CHR Director, Home Health CMA, Community Health Representatives, healthcare providers, Health Services programs, patients, families, caregivers, and community resources to support continuity of care, disease prevention, health maintenance, patient safety, and improved health outcomes.

The position requires professionalism, confidentiality, culturally responsive and patient-centered care, sound clinical judgment within the nurses authorized scope of practice, and timely communication with healthcare providers regarding significant changes in a patients condition.
