What Youll Do: As a Health Services Coordinator (LPN or RN), youll play a hands-on role in improving the lives of our residents by: Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners.Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
