Shared Living Service & Support Supervisor

Job Title: Shared Living Service & Support Supervisor Company: Vista Care Location: Hybrid / Field-Based Job Type: Full-Time The Role: Join Vista Care and empower individuals with developmental disabilities! As our Shared Living Supervisor, you will be the primary support system for our Independent Providers.

In this flexible hybrid role, youll guide, train, and oversee providers to ensure exceptional, person-centered care.

Key Responsibilities: Provider Support: Mentor providers, conduct QA audits, and ensure regulatory compliance.

Care Coordination: Drive person-centered care plans and coordinate client healthcare needs.

Field Visits: Conduct routine visits to Shared Living settings to ensure health and safety.

Crisis Management: Serve as an on-call resource for incident response and emergency support.

Requirements: 3+ years in human services (IDD experience preferred).

Bachelors Degree in Human Services OR 3 years of relevant equivalent experience.

Valid drivers license, reliable personal vehicle, and active auto insurance.

Ability to lift up to 50 lbs and safely assist with participant transfers.
