Shared Living Coordinator

Shared Living Coordinator Vista Care Job Type: Full-Time, Exempt Location: Hybrid / Field-Based (Up to 75% local travel) Denver, Aurora, Centennial area The Mission Are you a natural matchmaker with a passion for human services? Vista Care is seeking a driven Shared Living Coordinator to connect individuals with intellectual and developmental disabilities (IDD) to safe, empowering Host Homes.

In this dynamic, field-based role, you aren't just filling vacanciesyou are changing lives by building supportive families and inclusive communities.

Your Impact Recruit & Vet: Source, interview, and train compassionate Shared Living and Respite providers.

Assess & Match: Evaluate individual needs, facilitate meet-and-greets, and coordinate life-changing residential placements.

Inspect & Advocate: Ensure home safety (HUD/HQS standards), network with community agencies, and oversee the critical first 90 days of a new placement.

Manage Crises: Collaborate with nursing and leadership teams to coordinate rapid emergency placements when individuals are in urgent need.

What You Bring Education & Experience: High School Diploma required (Bachelors in Human Services and 3+ years of IDD experience strongly preferred).

Requirements: 18+ years old, valid drivers license, clear driving record, and ability to pass extensive background checks.

The "Fine Print": This role requires up to 75% in-state travel, flexibility for variable hours/on-call rotation, and the physical ability to lift 50 lbs and safely assist with individual transfers (up to 250 lbs using equipment/two-person assist).
