Know that you make a difference everyday helping others live their best life.Owns and manages each resident's comprehensive care and active treatment plan.Leads the interdisciplinary team, including the individual, family members, DSPs, and medical/therapy specialists.Monitors day-to-day results, evaluates services outcomes, maintains detailed administrative records, writes Individual Services Plans (ISPs).Ensures home and care practices meet state and federal standards, such as Medicaid requirements.Acts as a central bridge connecting families, medical staff, and care providers.Use your mentoring/coaching skills to teach people self-reliance.
