Position Summary

As an NHC Registered Dietitian, you have a vital and valued role in our patients' lives.

You will assess patients' nutritional needs; develop, implement, and monitor care plans; and ensure compliance with federal, state, and center guidelines.

Success in this role requires strong communication, food systems management knowledge, and interdisciplinary collaboration.

Responsibilities


* Complete and document admission, quarterly, annual, significant-change, and high-risk patient nutrition assessments


* Develop and monitor individualized nutrition care plans


* Provide nutrition education and training to food service and center staff


* Collaborate with nursing, therapy, administration, and food and nutrition services to promote optimal patient outcomes


* Participate in care plan meetings and interdisciplinary committees


* Lead Quality Assurance and Performance Improvement (QAPI) initiatives, conduct audits, and implement action plans for continuous improvement


* Complete MDS assessments and documentation
