Social Services Director (Full-Time)

Sunnyvale Gardens Post Acute is seeking a dynamic, compassionate, and experienced Social Services Director to lead our department.

Compensation & Benefits


* Salary Range: $75,000 - $100,000 annually (depending on experience)


* Job Type: Full-Time

Key Responsibilities


* Department Leadership: Oversee the day-to-day operations of the Social Services Department, providing guidance and supervision to assistant staff.


* Care Planning & Assessment: Conduct comprehensive psychosocial assessments, develop individualized care plans, and participate in interdisciplinary team meetings.


* Discharge Planning: Manage complex discharge planning processes, ensuring safe, seamless transitions of care back into the community or lower acuity settings.


* Regulatory Compliance: Maintain full compliance with federal, state (CDPH), and local regulations governing long-term care social services.


* Family & Resident Relations: Act as the primary liaison and advocate for residents and families, addressing grievances, rights concerns, and financial resource guidance (Medi-Cal/Medicare).

Qualifications & Requirements


* Education: Bachelor's degree in Social Work (BSW) or a human services field is required; Master's degree in Social Work (MSW) is highly preferred.


* Experience: Minimum of 2-3 years of social services experience in a skilled nursing facility (SNF), post-acute rehab, or long-term care setting.


* Leadership Skills: Proven experience managing a department or supervising staff within a healthcare environment.


* Technical Skills: Proficiency with electronic health record (EHR) software (e.g., PointClickCare) and regulatory documentation.
