🌟 What You'll Do

📝 Coordinate and complete MDS assessments accurately and timely

🤝 Collaborate with nursing, therapy, dietary, and social services teams

📊 Ensure compliance with Medicare, Medicaid, and regulatory requirements

📅 Monitor assessment schedules and care planning processes

📈 Assist in maximizing reimbursement while maintaining quality resident care

🔍 Review clinical documentation for accuracy and completeness

💙 Advocate for residents by ensuring assessments reflect their care needs

🎯 Qualifications

✔ Current, active RN license

✔ Previous MDS experience required

✔ Strong understanding of Medicare, Medicaid, and reimbursement processes

✔ Knowledge of RAI guidelines and care planning

✔ Excellent organizational and communication skills

✔ Ability to work independently and as part of a team

✔ Long-term care/skilled nursing experience preferred

General Purpose

The primary purpose of your job position is to oversee the facility to ensure assessments are completed per guidelines.

A primary focus is the resident assessment process, which includes individual assessment, MDS assessment, and resident plan of care.

Essential Duties

• Provides oversight on facilities' weekly PDPM meetings for skilled patients.

• Tracks and monitors facility effective utilization of PDPM tools (including but not limited to Physician Diagnosis Verifica...
