Hours: Full-Time 40 hrs a week (5/8s) 

Job Summary:

The physical therapist evaluates, plans, and implements rehab programs to enhance mobility, alleviate pain, boost strength, and prevent or correct deformities.

Services are delivered cost-effectively, with a focus on quality and customer satisfaction, while adhering to all regulatory standards.

Core Responsibilities and Essential Functions:

Patient Assessment:

  • Collaborate with physicians to develop, evaluate, and update treatment programs, care plans, goals, and discharge plans based on the patient’s physical therapy needs.

  • Create and adjust care plans with input from the patient and/or family.

  • Develop realistic discharge plans incorporating patient and family goals.

Treatment Planning and Provision of Care:

  • Implement and adjust physical therapy treatment plans.

  • Select appropriate activities to help patients achieve their goals.

  • Revise goals and plans with input from patients and families.

  • Provide ongoing education and training to patients and families.

  • Give discharge instructions and referrals to community resources as needed.

  • Serve as a resource for staff, families, and physicians regarding rehabilitation services and equipment.

Documentation:

  • Document evaluations, treatment plans, and each session according to departmental guidelines.

  • Complete and submit all required documentation, including family education and discharge information.
