Pleasant Hill Post Acute is seeking a compassionate, dependable, and clinically skilled Registered Nurse (RN) to join our team on a Part-Time / On-Call basis.

The ideal candidate is committed to providing high-quality, resident-centered care and working collaboratively with residents, families, physicians, and the interdisciplinary healthcare team.

Responsibilities


* Provide skilled nursing care to residents in accordance with individualized care plans.


* Administer medications and treatments as ordered.


* Monitor residents' conditions and promptly report changes in health status.


* Perform nursing assessments and assist with care planning.


* Maintain accurate and timely clinical documentation.


* Communicate effectively with residents, families, physicians, and healthcare staff.


* Follow infection control, safety, and facility policies and procedures.


* Promote resident dignity, comfort, safety, and overall well-being.


* Respond appropriately to changes in residents' conditions and emergency situations.

Qualifications


* Current and valid California Registered Nurse (RN) license.


* Previous skilled nursing or post-acute experience preferred.


* Strong clinical, communication, and organizational skills.


* Reliable, compassionate, and team-oriented.


* Ability to work independently and effectively in a fast-paced healthcare environment.
