We are looking for a skilled and dependableRegistered Nurse (RN) to serve as our Primary On-Call Home Care Nurse.

This position provides after-hours clinical support to patients, families, and field staff while helping ensure continuity of high-quality care throughout the evening and overnight hours.

This is a great opportunity for an experienced nurse who enjoysautonomy, clinical decision-making, and a flexible after-hours role.

The position is salaried at 24 hours per week, with the RN serving as the primary on-call resource during assigned hours.

This is an afterhours on-call role, with active work hours and patient visits varying based on patient and staff needs.

What You'll Do: Serve as the primary on-call nurse for home care services during assigned hours Respond to patient, family, and staff calls and provide appropriate clinical guidance Assess patient needs and determine appropriate interventions, including coordinating urgent or emergent care when necessary Provide clinical support and direction to field staff Coordinate after-hours visits and other services as needed Communicate with physicians, providers, pharmacies, and other members of the care team Respond to changes in patient condition and help determine appropriate next steps Complete necessary documentation and maintain accurate, timely records Provide a thorough transition of information to the daytime team Follow the Nurse Practice Act, agency policies, and applicable home health regulations What You Bring: Current NH and Maine Registered Nurse (RN) license or other compact state license Minimum 1 year of clinical nursing experience preferred; home care experience a plus Strong clinical assessment, critical-thinking, and decision-making skills Excellent communication and organizational skills Ability to work independently and confidently in an after-hours environment Comfortable managing multiple calls and priorities while remaining calm and professional Reliable transportation and valid driver's license Schedule & Compensation MondayThursday, 4:30 PM8:00 AM Salaried at 24 hours per week Serve as the primary on-call nurse during assigned hours The role is primarily on-call, with active work hours varying based on patient and staff needs After-hours visits will be required based on patient needs Why Cornerstone VNA?
