Senior Care Nurse

Senior Care Nurse (LPN) Fall River Health Services is seeking a Senior Care Nurse (LPN) to coordinate clinical care and provider services for residents in long-term care/skilled nursing and memory care settings.

This position serves as a clinical liaison between residents, families, nursing staff, providers, the clinic, pharmacy, and ancillary services.

The Senior Care Nurse supports provider rounds and visits, coordinates orders and referrals, maintains clinical documentation, and helps ensure residents receive timely, safe, and coordinated care.

The position works within the South Dakota LPN scope of practice, under appropriate supervision, and in accordance with organizational policies and regulatory requirements.

Key Responsibilities Coordinate resident provider visits and maintain schedules in accordance with CMS, payer, and organizational requirements.

Review resident records in EPIC and PCC to identify required physician and advanced practice provider visits and assessments.

Monitor required visit timelines and ensure residents are seen by the appropriate provider within required timeframes.

Collaborate with charge nurses and interdisciplinary team members to identify residents needing evaluation during provider rounds.

Coordinate laboratory, diagnostic, imaging, and other ancillary services needed for provider visits.

Prepare resident charts for provider rounds, including chart review, medication reconciliation, and identification of follow-up needs.

Room residents and obtain vital signs and other pertinent clinical information prior to provider evaluation.

Support providers during rounds by coordinating workflow, communication, and resident information.

Process and enter provider orders in accordance with established protocols, facility policies, and LPN scope of practice.

Coordinate specialty referrals, imaging, prior authorizations, podiatry services, and other specialty care.

Accurately document resident encounters, admissions, Medicare certifications, and other required clinical information in EPIC and PCC.

Communicate provider orders and clinical updates to nursing staff and other members of the care team.

Notify residents' families and/or designated representatives of significant clinical updates, care plan changes, and new provider orders as appropriate.

Support medication reconciliation and medication-management processes within the applicable care setting and LPN scope of practice.

Participate in care coordination, referral management, and transitions of care.

Maintain compliance with applicable clinical, regulatory, and organizational requirements.

Perform other clinical and administrative duties or special projects as assigned.

Qualifications Current Licensed Practical Nurse (LPN) license in South Dakota.

Experience in long-term care, skilled nursing, assisted living, memory care, clinic, or a related healthcare setting preferred.

Strong understanding of clinical documentation, medication management, and care coordination.
