Hospice On-Call RN - After Hours We are seeking an experienced Hospice Registered Nurse (RN) to provide on-call nursing support to hospice patients and families during after-hours, evenings, nights, and weekends.

This position is responsible for responding to patient and caregiver needs outside of regular business hours, including telephone triage, symptom management, urgent visits, and coordination of care.

The ideal candidate is an experienced hospice nurse who is comfortable working independently and making sound clinical decisions in an on-call environment.

What You'll Do Provide after-hours on-call nursing support for hospice patients and families Respond promptly to patient and caregiver calls regarding changes in condition, symptoms, medications, or other clinical concerns Provide telephone triage and determine when an in-person nursing visit is needed Make after-hours patient visits as clinically appropriate Assess and manage pain, symptoms, and changes in patient condition Provide education, guidance, and emotional support to patients and caregivers Coordinate with physicians, pharmacies, facilities, and other members of the hospice team as needed Communicate significant changes in patient condition to the appropriate clinical team members Complete timely and accurate documentation of calls, visits, assessments, and interventions Support patients and families during end-of-life situations Ensure care is provided in accordance with hospice regulations, company policies, and clinical standards What We're Looking For Active Registered Nurse (RN) license required Hospice nursing experience required Strong hospice clinical knowledge, including pain and symptom management Previous on-call, after-hours, or triage experience preferred Strong clinical judgment and ability to make independent decisions Excellent communication and organizational skills Ability to respond appropriately to urgent patient and caregiver needs Comfortable traveling to patient homes and facilities during assigned on-call hours Valid driver's license and reliable transportation Current CPR certification
