San Bruno Skilled Nursing is seeking a compassionate and experienced Social Services Director to join our dedicated team.

In this role, you will advocate for residents' social, emotional, and psychosocial well-being while collaborating with families, healthcare providers, and community resources to ensure the highest quality of care.

Key Responsibilities


* Assess residents' social, emotional, and family needs and develop individualized care plans.


* Assist residents in achieving the highest practicable level of independence and well-being.


* Provide medically related social services to support residents' physical, mental, and psychosocial health.


* Offer counseling and emotional support to residents and families coping with illness, disability, grief, and life transitions.


* Coordinate community resources and agency referrals to meet resident needs.


* Assist residents and families with healthcare decisions and discharge planning.


* Coordinate outpatient appointments and ancillary services, including dental, optometry, podiatry, and psychiatric care.


* Maintain accurate and timely social services documentation.


* Collaborate with interdisciplinary team members to ensure comprehensive resident care.


* Assist with resident belongings management and related concerns.


* Provide staff education and in-service training as needed.


* Foster positive relationships with residents, families, staff, and community partners.

Qualifications


* Bachelor's Degree in Social Work, Human Services, or a related field required.


* Minimum of 2 years of supervised social work experience in a healthcare setting.


* MSW preferred.


* Skilled nursing or long-term care experience preferred.


* Experience with PointClickCare (PCC) preferred.


* Excellent communication, organizational, problem-solving, and interpersonal skills.


* Ability to maintain confidentiality and exercise professional judgment.

Physical Requirements


* Frequent standing, walking, reaching, pushing, and pulling.


* Ability to lift up to 25 pounds.


* Occasional stooping, kneeling, crouching, balancing, and climbing.
