Position Summary

We are seeking a dedicated Physical Therapist (PT) to join our in-house rehabilitation team.

In this role, you will evaluate residents, develop individualized treatment plans, and provide skilled therapy services designed to improve mobility, strength, function, and independence.
Key Responsibilities


* Evaluate residents and develop individualized treatment plans


* Provide skilled physical therapy services in accordance with physician orders


* Monitor resident progress and adjust treatment plans as needed


* Collaborate with physicians, nursing staff, families, and interdisciplinary team members


* Complete accurate and timely documentation


* Participate in care plan meetings and discharge planning


* Educate residents and caregivers on exercises, safety, and mobility strategies


* Maintain compliance with all regulatory and professional standards


* Support positive resident outcomes and quality-of-life goals

Qualifications


* Available Sunday through Thursday (weekend availability required)


* Degree from an accredited Physical Therapy program


* Current South Carolina Physical Therapist license in good standing


* Long-term care or skilled nursing experience preferred


* Experience with PointClickCare (PCC) and Casamba preferred


* Strong clinical, communication, and organizational skills


* Must maintain all required continuing education and licensure requirements
