Job Description Job Title: Case Manager Reports To: Residential Manager Last Modified: 09.29.26 Department: Programs Program: Transitional Living Sub-Program: n/a FLSA Classification: Non-Exempt/Hourly Salary Range: $16.00-$17.00/hr.

Position Status: Full-time Expected Hours of Work: Sunday through Saturday; Day, Evening, Midnight Travel required: Some local travel Job Summary: The Case Manager is responsible for planning, coordinating and ensuring the delivery of necessary and appropriate services for assigned clients within established timeframes.

This employee will provide on-going assessment of youth, amend Life Skills Plans, serve as an advocate for clients, and provide program outreach.

This employee will ensure compliance with safe work practices and maintain safe environments in compliance with established agency standards, licensing regulations and generally accepted practices.

Duties and Responsibilities: Conduct Social and Life Skill Assessments.

Develop the Life Skill Plan.

Work in collaboration with the Planning Team including youth.

Meet with youth to teach Life Skills and assess progress.

Link and refer to necessary resources.

Obtain all necessary consents and release of information.

Advocate for youth.

Outreach responsibilities.

Case management as needed.

Attend required training.

Other duties as assigned.

Required Skills/Abilities: Ability to maintain clear personal and professional boundaries Experience working with youth and community members Ability to work effectively in constantly changing and sometimes demanding or chaotic environment.

Experience working with a diverse population, with sensitivity to differences in culture, religion, sexual orientation, race, age and physical and mental illness.

Strong public speaking skills Ability to communicate well both orally and in written form, and follow detailed instructions.

Knowledge of community resources Computer skills including the use of Microsoft office Valid Drivers License Education and Experience: Requires Bachelor degree in social work, psychology, sociology or related human service field; requires experience working with at-risk youth; prefers Life Skill instruction experience and case management Must have valid drivers license and proof of automobile insurance as required by law Work Environment: Business CasualResidential Physical/Environmental Demands: The table below shows how much on-the-job time is spent in the following physical activities.
